Job summary

The Staff Pad, in the Dallas area, seeks an experienced RN Case Manager to deliver compassionate hospice care in patients' homes. You will coordinate individualized care plans with patients, families, and the interdisciplinary team (IDT) while acting as an advocate for high-quality, patient-centered care.

Qualifications

• Graduate of an accredited Nursing program (Diploma, ADN, or BSN).
• Active Texas RN license required; Current CPR certification.

Responsibilities

• Perform comprehensive patient assessments and develop individualized care plans.
• Deliver skilled nursing care in patients' homes in accordance with hospice standards.
• Coordinate care with patients, families, physicians, and the IDT (interdisciplinary team).
• Supervise Hospice Aides and LVNs to ensure continuity of care.
• Maintain accurate, compliant clinical documentation.
• Complete patient admissions and participate in PRN on-call rotation.
• Manage patient schedules and prioritize changing care needs.
• Promote infection control, patient advocacy, and agency values.
• Assist with training and mentoring new clinical staff as needed.
• Commit to ongoing professional growth and clinical excellence through continuous education.
