Center4Hospice is seeking a Bereavement Counselor (LSW/LCSW) to support individuals and families navigating grief, with locations in Elkhart and Mishawaka, Indiana. This full-time, day-shift role offers independence within an interdisciplinary hospice team.

Candidates must hold Indiana LSW or LCSW; Michigan license is a plus. You will provide individual and group counseling, complete timely documentation, and contribute to community outreach and bereavement program development.

Qualifications

• Active Indiana LSW or LCSW license (or eligibility).
• Michigan license is a plus.
• Strong clinical assessment and communication skills.

Responsibilities

• Assess bereavement needs and complete risk assessments within 31 days of death.
• Develop and revise care plans for bereaved individuals seeking counseling.
• Provide individual, family, and group bereavement counseling and supportive calls.
• Maintain timely documentation and reporting within 24 hours of contact.
• Collaborate with an interdisciplinary hospice team and community outreach.
• Participate in planning and delivering bereavement services and education.
