Job summary

Qualifications

• Master’s degree required; LMSW license preferred.
• Minimum of one year grief support work experience in a recognized agency or health care organization.
• Ability to facilitate grief support groups and provide education on grief and loss.
• Ability to support people from varied cultures, ages, beliefs and communities with respect and empathy.
• Proven organizational, communication and follow-through skills; confidential information handling.

Responsibilities

• Design, develop and implement grief support services and educational programs.
• Provide interventions, including education and supportive counseling, to the bereaved client.
• Collaborate with patient care staff to ensure continuity of care for bereaved families.
• Develop and document bereavement plans of care reflecting family needs.
• Design and evaluate community-based grief support programs and presentations.
