Job summary

Affinity Hospice seeks a Licensed Masters Social Worker to assess psychosocial needs of patients and families, deliver counseling, and coordinate with the interdisciplinary team. This role emphasizes supportive counseling, casework, and pre-bereavement care.

Qualifications

• MSW degree and LMSW licensure in the state of employment.
• Clinical experience or training in death, dying, and bereavement.
• Knowledge of hospice regulations and ability to work a flexible schedule.
• Valid driver's license and auto liability insurance.

Responsibilities

• Provide psychosocial services to terminally ill patients and their families.
• Regularly assess patient and family psychosocial needs and adapt plans.
• Attend weekly interdisciplinary group meetings and participate in on-call rotation.
• Assist with funeral planning, financial and health care decisions as needed.
