Job summary
Puget Sound Home Health & Hospice in Washington state is seeking qualified Home Health Registered Nurses (RNs) to join our expanding team. You will assess patients in their homes, develop personalized care plans in collaboration with providers, and deliver skilled nursing care and education to patients and families.
Ideal candidates hold an active RN license, BLS certification, and have Home Health or Hospice experience.
Qualifications
- Current RN license
- Current AHA BLS certification
- Home Health and/or Hospice experience preferred
- Valid driver's license and insured vehicle
- Ability to work independently while collaborating with a clinical team
Responsibilities
- Assess patients in their homes and identify their individual care needs
- Develop and manage personalized plans of care in collaboration with providers
- Provide skilled nursing care, education, and support to patients and families
- Coordinate care with therapists, physicians, and other members of the interdisciplinary team
- Monitor patient progress and adjust care as needs change
- Complete accurate, timely clinical documentation
- Participate in quality initiatives that help us continually improve patient care
