Job summary

Job Summary

Performance Home Health is seeking an experienced and compassionate Home Health - Registered Nurse RN to join our growing team serving patients throughout the Dallas-Fort Worth Metroplex, including Dallas County, Collin County, Denton County, and Tarrant County, with opportunities in additional service areas as we continue to grow. This is a remote, field-based opportunity.

Responsibilities

• Complete Start of Care (SOC), recertifications, and discharge assessments
• Develop, implement, and update individualized plans of care in collaboration with physicians and care teams
• Provide wound care and wound vac management
• Provide PICC and port care
• Provide IV therapy and injections
• Provide Patient and caregiver education
• Monitor patient condition and communicate changes to physicians and clinical leadership
• Coordinate care with interdisciplinary team members (therapists, case managers, staffing)
• Educate patients and families on disease management, medications, and self-care
• Adhere to HIPAA, infection control, and safety standards
• Participate in case conferences, in-services, and quality improvement initiatives

Productivity & Visit Expectations

• Meet weekly productivity (30 points) and visit expectations
• Maintain timely completion of all documentation
• Communicate proactively regarding scheduling challenges or missed visits
• Support coverage needs to ensure continuity of care

Required Experience

• Active RN license in the state of Texas
• Minimum of 2 years of RN experience
• Must have at least 1 year of home health experience
• Wound care and/or wound vac certification
• IV therapy or infusion experience
• PICC line and implanted port care experience
• IV therapy administration and injectable medication administration experience
• Strong clinical assessment and critical thinking skills
• Able to work independently in the field
• Proficient with EMR systems
• Understanding of home health documentation and regulations
• Valid driver’s license and reliable transportation
• Strong organizational and time management skills
• Knowledgeable of HIPAA compliance

Preferred Experience

• Experience with KanTime EMR
• LVAD experience
• Bilingual (preferred but not required)
• Remote (field-based position. RN will work in the field and travel to patients' homes rather than to a central office.)
• Independent home visits across assigned territory

Salary Compensation

Pay: $100,000
