Job summary

VALLEY HOME HEALTH CARE INC is seeking a dedicated Physical Therapist to deliver in-home assessments, plan of care, and evidence-based interventions for patients in Las Vegas. You will collaborate with physicians and care teams, document care in EMR, and contribute to quality improvement in a dynamic home health setting.

Qualifications

• Graduate of an accredited Physical Therapy program.
• Current active Physical Therapist license in the practicing state.
• CPR certification required.
• Reliable transportation and valid driver’s license.

Responsibilities

• Perform initial and ongoing physical therapy assessments to determine patient needs and establish plan of care.
• Develop and revise individualized treatment plans in collaboration with the patient, physician, and other care team members.
• Deliver evidence-based physical therapy interventions including gait training, strengthening, pain management, balance improvement, and fall prevention.
• Monitor patient progress and adjust treatment plans as necessary.
• Educate patients and caregivers on exercises, home safety, and use of assistive devices to promote optimal function and independence.
• Document all care provided in EMR systems according to guidelines.
• Communicate effectively with physicians, nurses, and interdisciplinary team members to coordinate patient care.
• Participate in case conferences and team meetings as required.
• Supervise PTAs and other staff according to scope and regulatory requirements.
• Maintain professional licensure and comply with continuing education requirements.
• Adhere to infection control and safety standards during home visits.
• Participate in quality improvement and performance management activities.
