Qualifications

• Knowledge of durable medical equipment (DME) is required.

Responsibilities

• Plan, implement and evaluate patient care plans to restore or maintain patient well-being.
• Plan, organize and conduct physical therapy treatment programs based on medical referrals and evaluations.
• Collaborate with interdisciplinary team to coordinate care.
