FeldCare Connects in El Monte is seeking a self-motivated Physical Therapist to deliver premier home health care to adults of all ages as an independent contractor.

The PT will assess patient needs, develop and execute a treatment plan, monitor progress, and maintain accurate records, with flexible scheduling and autonomy.

We provide resources to support independence and work-life balance. Successful candidates will have state licensure and the required PT curriculum.

Qualifications

• Licensed physical therapist in the state
• Experience with adults of all ages
• Knowledge of rehabilitation principles and practices

Responsibilities

• Assess and evaluate patient care needs to promote movement and function
• Determine and implement a treatment plan and monitor progress
• Document and maintain patient records with accuracy and discretion
• Adjust treatment plans as needed based on patient progress
