Job summary

Welltech Partners is seeking a committed Registered Physical Therapist for home health in the Seattle, WA area. You will deliver physician-prescribed therapy in patients’ homes, following the care plan, and work under the supervision of Clinical Services to maximize functional outcomes.

The role emphasizes autonomy, one-on-one patient relationships, and contributing to enhanced home health care across King County.

Qualifications

• Licensed PT with CAPTE accreditation.
• Current WA PT license in good standing.
• Minimum 2 years of PT experience; home health experience a plus.

Responsibilities

• Conduct initial assessments and develop care plans; reassess every 30 days.
• Provide physician-prescribed PT in patients' homes.
• Complete and submit OASIS and related documentation on time.
• Instruct patients and families in home exercise programs.
• Supervise PTAs and HHAs as assigned and participate in care conferences.
