Job summary

CenterWell Home Health in Seattle is seeking a Home Health Physical Therapist to provide in-home PT services, improving patients’ strength, mobility and independence. You will work under the Clinical Manager to deliver personalized care in patients’ homes and coordinate with a multidisciplinary team.

The ideal candidate holds a PT license, CPR certification, and has experience collaborating with healthcare professionals. Home health experience and a Doctorate in Physical Therapy are preferred.

Qualifications

• Current and unrestricted Physical Therapy license.
• Current CPR certification.
• Experience collaborating with a team of healthcare professionals.

Responsibilities

• Plan and administer prescribed physical therapy in patients' homes to promote best function.
• Assess and document patients' physical strengths to guide treatment goals.
• Develop and implement rehab programs aligned with physician plans and patient goals.
• Provide PTAs and Home Health Aides with clear written care instructions and supervision.
• Document patient observations, interventions, and evaluations promptly and thoroughly.
• Report patient progress to physician and care team and submit required documents.
