Job summary

CenterWell Home Health is seeking a Licensed Physical Therapist to deliver in-home care in Everett, WA, under the Clinical Manager. You will promote functional improvement, help patients regain strength, mobility and independence, and coordinate with the care team.

This role requires an unrestricted PT license, CPR certification, and teamwork experience. The position offers 40 hours weekly with a competitive pay range and Humana benefits.

Qualifications

• Current and unrestricted Physical Therapy license.
• Current CPR certification.
• Experience collaborating with a team of healthcare professionals.

Responsibilities

• Plan and administer prescribed physical therapy in patients' homes to attain highest physical function.
• Assess and document patients' physical strengths to support physician evaluation and guide goals.
• Develop and implement rehab programs aligned with Plan of Treatment and patient goals.
• Provide PTAs and Home Health Aides with clear care instructions and oversee documentation.
• Document patient observations, interventions and evaluations promptly.
• Report progress to physician, Clinical Manager and care team; submit evaluations and discharge summaries.
