Job summary

AdventHealth in Temple Terrace, FL is seeking a Physical Therapist to provide comprehensive PT care in home health within Hillsborough, Pasco, Pinellas, Polk, and surrounding counties. The role involves evaluating patients, creating and implementing care plans, and educating patients and families.

Travel between covered counties is typical with mileage reimbursement. The position requires DPT or PT licensure with CPR/BLS and a Florida license.

Responsibilities

• Admits new patients and performs comprehensive initial evaluations.

• Instructs patients and families in the PT program and assistive devices.
• Participates in team conferences and school/home health coordination.
• Maintains clinical records and participates in quality improvement activities.
