AdventHealth in Temple Terrace, FL is seeking a licensed Physical Therapist to deliver patient-centered home health care. You will develop and reassess care plans, coordinate with physicians, and support the patient’s home rehabilitation goals.

This role requires a Doctorate (preferred) and a valid PT license, with experience in Medicare-certified home health settings being a plus. Schedule is typically weekdays with on-call elements in Hillsborough County.

Qualifications

Education requirements align with licensure for Physical Therapists; Doctorate preferred.
Current Physical Therapist (PT) license and related certifications required by state practice.
Experience in home health care and Medicare-certified settings preferred.

Responsabilités

Develops patient-specific care plans considering physical, cultural, psychosocial, spiritual, age-specific, and educational needs.
Reassesses patients, documents progress, and evaluates therapy effectiveness.
Communicates changes to physicians and trains patients and caregivers on device use and home programs.
