Job summary

BayCare HomeCare in Tampa, FL seeks a Home Health Physical Therapist to provide hands-on care in patients' homes, evaluating needs and developing treatment plans. The role includes directing PT assistants, coordinating care, and ensuring quality outcomes within a supported BayCare HomeCare team.

The position offers a flexible schedule, opportunity for growth, and includes weekend rotation. Requires Florida PT license, BLS, and valid FL driver’s license; home visits cover Hillsborough County.

Qualifications

• BLS certification required.
• PT License in Florida or eligible to obtain.
• Florida driver's license within 30 days of hire.

Responsibilities

• Evaluates and develops specific treatment programs for patients in home health.
• Provides clinical direction for Physical Therapy Assistants.
• Manages and coordinates home health services to meet patient needs and quality care standards.
• Directs activities of the multi-disciplinary team delivering home care.
• Monitors therapeutic services and communicates with other disciplines.
• Develops discharge plans with patient and family.
