Job summary

TheKey is seeking a Licensed Practical Nurse (LPN) to serve home health clients across Fairfield County, CT. This per diem/on-call/part-time role offers independence and flexible scheduling, with compensation starting at $30.00 per hour, varying by client and shift.

Home visits include planning, wellness checks, and direct clinical care. Ideal candidates have 1–2 years of nursing experience, a CT LPN license, CPR/BLS certification, and access to reliable transportation.

Qualifications

• Active, unencumbered Connecticut LPN license.
• 1–2 years of clinical nursing experience; home health/geriatrics preferred.
• Current CPR / BLS certification.
• Flexible schedule with per‑diem/on‑call availability.
• Valid Connecticut Driver’s License, auto insurance.

Responsibilities

• Provide skilled nursing care, routine wellness visits, medication administration, wound care, and health monitoring.
• Observe and report changes in patient conditions to supervising staff.
• Accept per‑diem visits on an as‑needed basis across Fairfield County.
• Build rapport with clients and families and educate on health topics.
• Complete and submit accurate clinical notes in line with policies and HIPAA.
