Qualifications

Active LPN license in Connecticut.
1–2 years of clinical nursing experience; home health or geriatrics preferred.
Current CPR/BLS certification.
Flexible schedule; per-diem, on-call, and last-minute availability.
Valid CT Driver's License, reliable vehicle, auto insurance.
Proficient in using EHR for daily charting.

Responsibilities

Provide skilled nursing care, routine visits, med administration, and wound care per orders.
Monitor patient status and promptly report changes to RN/physician.
Accept per-diem assignments as needed across Fairfield County.
Engage clients and families; provide health education and support.
Complete timely clinical notes per agency policy and HIPAA.
Transition to extended coverage as demand grows.
