The position offers flexible on‑call shifts with priority for last‑minute assignments and opportunities to extend coverage as patient volume grows. Ideal candidates hold an active CT LPN license, 1–2 years of clinical nursing experience, CPR/BLS, and a reliable vehicle

Qualifications

• Active, unencumbered Connecticut LPN license.
• 1–2 years' clinical nursing experience; home health or geriatrics preferred.
• CPR/BLS certification current.
• Flexible schedule for per‑diem, on‑call, and last‑minute assignments.
• Valid CT driver's license, reliable vehicle, and auto insurance.
• Strong clinical assessment, communication, and EHR charting skills.

Responsibilities

• Provide skilled nursing care and routine wellness visits per physician orders.
• Monitor patient status and report changes promptly to supervising staff.
• Accept per‑diem assignments with flexibility for coverage gaps.
• Engage clients and families and educate on health topics.
• Complete timely clinical documentation in accordance with policy.
• Support future coverage opportunities as demand grows.
