Job summary

TheKey Home Health Services in Hartford County, CT is seeking a flexible Licensed Practical Nurse (LPN) for per diem home health visits. This role offers independence, meaningful patient care, and an on-call schedule with need-based assignments across the region.

Qualifications

• Active Connecticut LPN license
• 1–2 years clinical home health or geriatrics preferred
• Current CPR/BLS certification
• Flexible per-diem availability and last-minute assignments
• Valid Connecticut Driver’s License and auto insurance
• Proficient with EHR daily charting

Responsibilities

• Provide skilled nursing care, routine wellness visits, medication administration, wound care, and health monitoring per orders and plans of care
• Observe, record, and report changes in patient conditions to RN or physician
• Accept per-diem visits and respond to coverage needs across Hartford County
• Engage with clients and families; offer health education and empathetic support
• Complete and submit timely clinical notes per policy and HIPAA
• Transition into extended shift or block coverage as care volume grows
