Job summary

FOX Rehabilitation is seeking a licensed Physical Therapist to deliver one-on-one care to older adults in Paterson, NJ, in private homes and senior living communities. You will evaluate, plan, and implement evidence-based therapy, promoting independence and mobility.

Role offers autonomy with scheduling flexibility and potential telehealth components. Full-time/part-time options with benefits, base pay range in NJ, and supportive, clinician-driven culture.

Qualifications

• Valid Physical Therapy license in state(s) of practice or eligibility to obtain.
• Degree from an accredited Physical Therapy program.
• Basic computer and EMR proficiency.
• Current CPR certification.

Responsibilities

• Provide one-on-one physical therapy to older adults in home or senior living settings.
• Perform evaluations, develop individualized plans of care, and deliver evidence-based treatment.
• Deliver therapy sessions to support wellness, mobility, and independence.
• Document effectively within FOX’s EMR system per regulatory standards.
• Collaborate with PTs, OTs, SLPs, and Fitness Specialists.
• Educate patients, caregivers, and staff to promote safety and aging in place.
• Provide telehealth/virtual care when appropriate.
