Job summary

Gracehaven is seeking an independently licensed behavioral health professional to serve as a Contracted Clinical Assessor for children and adolescents in residential, congregate-care, or community-based settings. The assessor delivers trauma-informed evaluations, clinical formulations, and placement recommendations.

The role emphasizes independent clinical judgment, confidentiality, and timely documentation, without routine supervision or replacement of the youth’s treating clinician.

Qualifications

• Master’s or doctoral degree in a relevant behavioral health discipline from an accredited institution.
• Current, independent professional license in good standing appropriate to the services provided.
• Experience assessing and working with children and adolescents.

Responsibilities

• Conduct trauma–informed, developmentally appropriate mental health assessments.
• Review referral information, medical and educational records, and collateral sources.
• Interview youth and obtain information from parents, guardians, and staff as appropriate.
• Evaluate functioning, trauma exposure, and safety concerns to inform level of care.
• Provide clinical recommendations to support treatment planning and safety planning.
• Document assessments clearly, objectively, and within contractual timeframes.
• Collaborate with treatment teams and other professionals to coordinate care.
