Job summary

Achieve Behavior Alliance Plus, LLC provides Home and Community Support Professionals to deliver individualized care for clients with intellectual or developmental disabilities. Qualifications

• Strong communication and interpersonal abilities.
• Professionalism, reliability, and independent problem-solving.
• Ability to adapt to changing client needs.
• Capacity to maintain accurate documentation.
• Familiarity with EVV and electronic documentation.
• Previous DSP or caregiving experience.

Responsibilities

• Provide direct care support, including meal preparation, mobility assistance, personal hygiene, and independence-building tasks
• Follow the client’s ISP to deliver skill-building, communication support, community participation, and social/recreational activities
• Document accurate session notes
• Report any changes in client behavior, health, or well-being
• Maintain professional communication with families, coordinators, and other providers
• Support a safe, respectful, and inclusive environment while upholding client dignity and privacy
• Coordinate scheduling directly with clients/families
