Job summary

The Moore Center is seeking a per diem direct support professional in New Hampshire to support high-risk/high-need individuals toward independence in all settings. Pay is $20/hr, with responsibilities including daily living assistance, behavior support, documentation, and transportation for residents.

The role requires adherence to state regulations and Moore Center policies, with ongoing training andRecord-keeping duties.

Qualifications

• Experience with high-risk/high-need individuals preferred.
• Ability to maintain detailed records and documentation.
• Strong communication with clients, supervisors and stakeholders.

Responsibilities

• Follow program schedule and daily duties as outlined by team and program manager.
• Implement resident programming and daily living skills per goals and objectives.
• Maintain compliance with residential standards and regulations per state and Moore Center.
• Complete documentation and report writing accurately and timely.
• Assist individuals with activities of daily living and encourage independence.
• Ensure health and safety of clients and supervise medication administration as applicable.
• Deliver transportation for residents in approved vehicles.
• Participate in training and meetings as required.
• Follow behavior plans and document interventions.
