Job summary

The Moore Center is seeking a Part-Time Direct Support professional to assist high-need individuals in achieving independence in daily settings. You will follow a set schedule, support daily living skills, and ensure safety and documentation standards are met.

Responsibilities include implementing behavioral plans, assisting with personal care, coordinating with clinical teams, and transporting residents as needed.

Qualifications

  • Bachelor's degree (or equivalent experience) plus minimum of 1 year experience in human services required; 3 years' experience preferred.
  • Valid driver’s license and access to a safe operating vehicle during scheduled working hours.
  • Criminal, motor vehicle, and related background checks as per agency standards.
  • MOAB training completion preferred.
  • Must adhere to mandated reporting requirements.

Responsibilities

  • Follow program schedule and daily duties as outlined by team and program manager.
  • Implement resident programming goals and daily living skills training.
  • Ensure compliance with Residential Standards and Regulations.
  • Maintain accurate records and timely documentation.
  • Communicate with clients, supervisory team, and stakeholders for seamless care.
  • Assist with Activities of Daily Living and support self-advocacy.
  • Ensure health and safety of clients and environment.
  • Supervise medication self-administration and administration as needed.
  • Complete behavior management training and document interventions.
  • Attend required trainings and meetings.
  • Implement behavior/risk management plans per HR and program guides.
  • Supervise medical/evaluative appointments and accompany residents to them.
  • Transport residents in approved vehicles.
  • Remain on shift until coverage is provided.
  • Assist with residence upkeep and equipment.

Description

Position Summary:

To professionally support high risk/high need individuals receiving services to achieve the highest level of independence in all settings in accordance with all applicable state and agency regulations and protocols.

PAY: $20/hr.
Schedule: Part Time - Sunday 3pm to 9pm, Tuesday 3pm to 9pm, Friday 7am to 9pm. 26 hours
Essential Responsibilities:
  • Follow program schedule and daily duties as outlined by team and program manager
  • Responsible for consistent implementation of resident programming training (Service Agreement Goals & Objectives: daily living skills
  • Responsible for meeting all Residential Standards and Regulations as required by the Division of Mental Health and Developmental Services and/or Moore Center Services, Inc.
  • Responsible for appropriate record keeping and report writing and timely/accurate documentation
  • Communicate effectively with clients, clinical supervisory team and other stakeholders to ensure seamless translation of knowledge
  • Assist individuals in Activities of Daily Living, including but not limited to providing personal care and ensuring appropriate attire is worn at all times. Encourage utilization of self-advocacy skills to foster independence
  • Ensure general health and safety of client(s) and all environments as assigned on a daily basis
  • Supervise residents’ self-administration and/or the distribution and administration of prescribed medications
  • Complete behavior management training programs prior to working with specific residents; consistently implement and document interventions as indicated in programs (see addendum for physical requirements)
  • Attend ongoing training and meetings as required
  • Implement behavioral interventions as guided by written and Human Rights Committee (HRC) approved behavior and risk management plans
  • Responsible for supervision of residents’ medical/evaluative appointments and accompaniment of resident to such appointments
  • Provide transportation for residents in vehicle approved by Agency
  • Responsible for remaining on shift until adequate coverage can be provided as determined by house manager or designated responsible person (Shift Leader)
  • Responsible for aiding in the general upkeep of residence, furnishings and equipment
  • Adhere to the NH state law requiring mandated reporting of suspected abuse, neglect, or exploitation
  • Other duties as assigned by supervisor
Requirements
Education and Experience
  • Bachelor's degree (or equivalent experience) plus minimum of 1 year experience in human services required; 3 years' experience preferred
  • $100,000/300,00 Automobile Liability Insurance
  • Valid driver's license and access to a safe operating vehicle during scheduled working hours
  • Criminal, motor vehicle, and Bureau of Elderly and Adult Services background checks that meet agency standards
  • Required state trainings through Relias Learning or equivalent preferred
  • Completion of Management of Aggressive Behaviors (MOAB) training part 1 and part 2
  • It is considered a bona fide occupational qualification (BFOQ) for direct support staff /professionals to accompany clients to religious ceremonies as dictated by the clients' personal religious preferences

  • Provide personal care and behavioral support, including but not limited to physical assistance
  • Maintain professional demeanor in emotionally charged situations
  • Ability to write detailed accounts of incidents and behaviors
  • Ability to successfully complete medication training if applicable.
  • Demonstrates flexibility in scheduling and daily assignments
  • Demonstrate proficiency in Microsoft Office, including Word, Excel, Outlook, and the ability to master new software

Staff are also required to travel and transport clients when applicable. This includes having access to a reliable vehicle, a valid driver's license and maintaining comprehensive personal auto insurance of $100,000/300,000.
