Job summary

Hireology is seeking a dedicated Occupational Therapist (OT) on a PRN basis to deliver person-centered care in a skilled nursing setting. You will enhance residents’ independence and safety through ADL/IADL activities and collaborative care planning.

Ideal candidates hold an active Texas OT license, with SNF or long-term care experience and familiarity with Net Health documentation. This role offers flexible schedules and competitive compensation in a supportive rehab team.

Qualifications

• Active and valid Texas Occupational Therapy license.
• In good standing with regulatory agencies and boards.
• SNF or long-term care experience preferred.
• Knowledge of occupational therapy treatment practices.
• Experience with Net Health Electronic Documentation System preferred.
• Ability to manage patients with varying personalities and needs.
• Strong communication, collaboration, and clinical decision-making skills.

Responsibilities

• Consult with patients to evaluate their condition and impact on daily occupations.
• Assess evaluations and test results and interpret findings.
• Develop individualized treatment plans to maximize ADLs/IADLs.
• Assess home environments and recommend modifications.
• Recommend adaptive equipment and provide training for independence.
• Deliver treatments per the established plan of care.
• Monitor response to treatment and adjust interventions with the team.
• Provide education to patients, families, and caregivers.
• Collaborate with interdisciplinary team to ensure quality care.
• Complete documentation promptly in compliance with policies and regulations.
• Attend required meetings designated by Rehab leadership.
