Job summary

Community Options, Inc. in Baltimore, MD is seeking a Full-Time Community Integration Counselor to help individuals with disabilities learn daily living, social, and community skills.

The role involves meeting waiver participants, providing ongoing community integration support, documenting progress, coordinating activities, transporting to appointments, and ensuring proper documentation and billing. Starting pay is $18.00 per hour.

Qualifications

• High school diploma or GED is required; a Bachelor’s degree is preferred.
• Two years of relevant experience supporting individuals with intellectual and developmental disabilities is required.

Responsibilities

• Support individuals to increase independence.
• Meet waiver participants at homes, in the community, or office settings.
• Provide ongoing support for community integration efforts.
• Document progress toward community integration.
• Coordinate leisure and volunteer opportunities.
• Administer medications and ensure documentation.
• Drive individuals to medical appointments and activities.
• Ensure timely and accurate program documentation and billable records.

Education
