Qualifications

• OD from accredited optometry school.
• Valid state optometry license.
• Ophthalmologist (MD or DO) from accredited school.
• Valid state ophthalmology license.
• General Liability Certificate.
• Professional Liability Insurance Certificate.
• W-9.

Responsabilités

• Provide eye and vision care and general health care to patients in Walmart stores.
• Establish days of practice and hours of operation.
• Control patient schedule and manage appointments.
• Set your fee schedule and retain 100% of exam/professional fees.
• Offer equipment/instrumentation and/or administrative/Opt-Tech support where state allows.
