Job summary

Graduate Post Acute is seeking an experienced MDS Coordinator to ensure timely and accurate completion of all MDS assessments in a busy long-term care setting in Philadelphia. You will supervise data entry, validate information, and prepare reports for leadership, ensuring compliance with Medicare/Medicaid requirements.

The role requires an active RN license and at least three years of LTC experience with prior MDS/RAI familiarity.

Qualifications

• RN license active and in good standing.
• Minimum of three years of clinical experience in long-term care.
• Prior MDS/RAI experience required.

Responsibilities

• Ensure timely and accurate completion of all MDS assessments per regulations and deadlines.
• Verify compliance with federal/state regulations related to MDS documentation and submission.
• Supervise MDS data entry, validation, and transmission processes.
• Identify and resolve data discrepancies and validation issues.
• Prepare and present MDS-related reports to the DON and leadership.
• Provide feedback to clinical teams on documentation and care planning.
• Support surveys, audits, and regulatory reviews with required documentation.
• Assist with audit responses and ongoing Medicare/Medicaid compliance.
• Stay current on Medicare, Medicaid, and RA I/MDS guidelines.
• Contribute to MDS quality improvement and interdisciplinary collaboration.
