Job summary

Northbrook Rehabilitation Center in Cedar Rapids, IA, is seeking a licensed Physical Therapist (PT) to join our in-house rehabilitation team. The role focuses on subacute rehabilitation and skilled nursing, helping residents regain strength, mobility, independence, and quality of life.

You will work with an interdisciplinary team to deliver patient-centered care. The ideal candidate should hold a Doctorate or Master’s in Physical Therapy, possess Iowa licensure or eligibility, and have CPR/BLS

Qualifications

• Doctorate or Master's degree in Physical Therapy from an accredited program.
• Licensed or eligible for Iowa Physical Therapist license.
• Current CPR/BLS certification.

Responsibilities

• Evaluate residents' physical, functional, and mobility needs.
• Develop individualized treatment plans based on clinical findings and resident goals.
• Provide skilled physical therapy services to improve strength, balance, coordination, and mobility.
• Assist residents recovering from surgery, illness, and other conditions.
• Provide gait, transfer, balance, therapeutic exercise, and functional mobility training.
• Educate residents and staff on mobility and safety.
• Complete timely clinical documentation.
• Participate in care planning and interdisciplinary team meetings.
