Job summary

Northbrook Rehabilitation Center in Cedar Rapids, IA is seeking a compassionate Occupational Therapist to join our in-house rehab team. You will work with adults in a subacute rehabilitation and skilled nursing setting to help residents regain independence and meaningful daily activities.

The ideal OT will collaborate across nursing, therapy, dietary, and medical staff, evaluate functional needs, and develop patient-centered treatment plans to improve ADLs and mobility.

Qualifications

• OT degree from an accredited program; licensure in Iowa or eligibility.
• Current Iowa OT license or eligibility.
• Current CPR/BLS certification.
• Experience in skilled nursing, subacute rehab, acute care, or related clinical settings preferred.
• Strong clinical, communication, organizational, and documentation skills.
• Ability to work effectively on an interdisciplinary rehab team.
• Compassionate, dependable, and resident-focused approach to care.

Responsibilities

• Evaluate residents' functional abilities, occupational performance, and rehabilitation needs.
• Develop individualized treatment plans based on clinical findings, resident goals, and functional needs.
• Provide skilled occupational therapy services to improve independence with ADLs and functional mobility.
• Address ADLs and IADLs, including dressing, bathing, grooming, toileting, feeding, transfers, and other functional activities.
• Provide therapeutic exercises, activities, neuromuscular re-education, and functional training as clinically appropriate.
• Assess and address upper-extremity strength, ROM, coordination, balance, and functional use.
• Educate residents, families, caregivers, and staff regarding safety, positioning, adaptive techniques, and strategies to maximize independence.
• Participate in care planning and interdisciplinary team meetings.
• Complete timely and accurate clinical documentation.
