Job summary

Evolve Therapy Services, LLC is seeking a Physical Therapy Assistant for an in-house Skilled Nursing Facility in Butler, AL. This PRN role supports the Physical Therapist and contributes to high-quality resident care.

The therapist will conduct screenings, assist in evaluations, help develop treatment plans, and provide both individual and group sessions. Documentation is completed in NetHealth EMR per CMS guidelines. Active PTA license in Alabama is required.

Qualifications

• Associate's degree in physical therapy from an accredited program.
• Active Physical Therapy Assistant license in state(s) of practice.
• Ongoing continuing education and professional development to maintain current licensure and certification.

Responsibilities

• Provide physical therapy services to residents under the direct supervision of the Physical Therapist.
• Conduct screenings of residents at regular intervals to determine need for intervention/treatment.
• Identify indications for evaluations and assist in evaluation under the direction of the Physical Therapist.
• Assist in developing treatment plans with short- and long-term goals.
• Deliver individual and group treatments focusing on strength, mobility, balance, gait, and ROM.
• Maintain documentation consistent with CMS expectations in NetHealth EMR.
