Job summary

Evolve Therapy Services is seeking a Licensed Physical Therapist to join our in-house Skilled Nursing Facility partner in Erie, PA. The role involves providing PT services, developing treatment plans, and collaborating with an interdisciplinary team to ensure high-quality resident care.

Responsibilities include patient evaluation, plan implementation, documentation in NetHealth EMR, and adherence to CMS guidelines.

Qualifications

• Bachelor of Science in Physical Therapy from an accredited program.
• Active Physical Therapy license in state(s) of practice.
• Ongoing continuing education and professional development to maintain current licensure and certification.

Responsibilities

• Provide physical therapy services to residents in accordance with established standards of practice and company procedures.
• Adhere to documentation and billing procedures for physical therapy services.
• Demonstrate sound judgment in evaluation, planning, implementation and follow-up of resident therapy programs.
• Conduct screenings to determine need for intervention/treatment.
• Develop treatment plans with short and long-term goals and methods to achieve them.
• Interpret and communicate evaluation findings and recommendations to residents, families and interdisciplinary team members.
• Complete compliant and medically necessary CMS documentation in NetHealth EMR.
