Qualifications

• Minimum Bachelor of Science in Physical Therapy from an accredited program.
• Active Physical Therapy license in state(s) of practice.
• Ongoing continuing education and professional development to maintain licensure.

Responsibilities

• Provide physical therapy services to residents per established standards and procedures.
• Adhere to documentation and billing procedures for therapy services.
• Develop and implement treatment plans with short- and long-term goals.
• Communicate evaluation findings and treatment plans to residents, families, and the interdisciplinary team.
• Complete CMS-relevant documentation in NetHealth EMR systems.
