Job summary

Evolve Therapy Services, LLC is hiring compassionate Physical Therapists for an in-house Skilled Nursing Facility in Erie, PA. The role offers full-time, part-time, or PRN opportunities and focuses on planning, implementing, and evaluating therapy plans to ensure high-quality resident care.

Responsibilities include screening, treatment planning, documentation, and CMS-compliant reporting in NetHealth EMR. Candidates must hold a BS in Physical Therapy and an active state license, with ongoing CE

Qualifications

• Minimum of Bachelor of Science Degree in Physical Therapy from an accredited program.
• Must have an active Physical Therapy license in state(s) of practice.
• Ongoing continuing education and professional development to maintain current licensure and certification.

Responsibilities

• Provide physical therapy services to residents in accordance with established standards of practice.
• Adhere to documentation and billing procedures for physical therapy services.
• Demonstrate sound judgment in evaluation, planning, implementation, and follow-up of resident therapy programs.
• Conduct screening of residents to determine need for intervention/treatment.
• Develop treatment plans with short and long-term goals and methods to achieve identified goals.
• Interpret and communicate evaluation findings and recommendations to residents, families, and the interdisciplinary team.
• Complete compliant and medically necessary CMS documentation in NetHealth EMR systems.
