Job summary

Evolve Therapy Services is seeking a Physical Therapist for an in-house Skilled Nursing Facility in Erie, PA. Full Time, Part Time, or PRN positions are available to support patient care and program goals.

You will evaluate residents, develop and implement treatment plans, and collaborate with an interdisciplinary team to provide high-quality care that meets CMS standards. Ongoing CEU participation is encouraged and licensure must be maintained.

Qualifications

• Bachelor of Science in Physical Therapy from an accredited program.
• Active Physical Therapy license in state(s) of practice.
• Ongoing continuing education to maintain licensure and certification.
• Commitment to maintaining current licensure and certification.

Responsibilities

• Provide physical therapy services to residents in accordance with established standards and procedures.
• Document and bill therapy services per company procedures.
• Demonstrate sound judgment in evaluation, planning, implementation and follow-up of therapy programs.
• Conduct screening of residents to determine need for intervention/tersonalized treatment.
• Evaluate residents to obtain data for treatment planning and implementation.
• Develop short and long-term treatment plans with goals and methods.
• Communicate findings and recommendations to residents, families and interdisciplinary team.
• Complete CMS-compliant documentation in NetHealth EMR systems.
