Job summary

Evolve Therapy Services, LLC seeks a qualified Physical Therapist for an in-house Skilled Nursing Facility in Hudson, OH. The role supports planning, implementing, and directing therapy services to residents per CMS and state standards.

Qualifications

• Bachelor of Science in Physical Therapy from an accredited program.
• Active Physical Therapy license in state(s) of practice.
• Ongoing continuing education to maintain licensure and certification.

Responsibilities

• Provide physical therapy services to residents per standards of practice and departmental procedures.
• Adhere to documentation and billing procedures for PT services.
• Demonstrate sound clinical judgment in evaluation, planning, and follow-up of therapy programs.
• Conduct regular resident screenings to determine intervention needs.
• Develop treatment plans with short- and long-term goals and methods to achieve them.
• Communicate evaluation findings and treatment recommendations to residents and families.
