Job summary

Evolve Therapy Services, LLC is seeking a Physical Therapist for an in-house Skilled Nursing Facility located in South Sioux City, NE. The role offers full-time hours with a $10,000 sign-on bonus and flexible scheduling, plus part-time or PRN options.

Responsibilities include planning, directing physical therapy services, documenting according to CMS expectations, and collaborating with an interdisciplinary team to ensure high-quality resident care.

Qualifications

• Bachelor of Science Degree in Physical Therapy from an accredited program.
• Active Physical Therapy license in state(s) of practice.

Responsibilities

• Provide physical therapy services to residents in accordance with standards, procedures, and productivity.
• Adhere to documentation and billing procedures for physical therapy services.
• Demonstrate sound judgment in evaluation, planning, implementation, and follow-up of resident therapy programs.
• Conduct screenings to determine need for intervention/treatment.
• Evaluate residents to obtain data for treatment planning and implementation.
• Develop treatment plans with short and long-term goals.
• Interpret and communicate evaluation findings and recommendations to residents and interdisciplinary team.
