Job summary

Evolve Therapy Services, LLC is hiring a Physical Therapist for an in-house Skilled Nursing Facility in South Sioux City, NE. This role offers full-time, part-time or PRN options with a $10,000 sign-on or relocation bonus.

Flexible scheduling supports a work-life balance while delivering quality resident care. The therapist will plan, organize, and direct PT services, maintain CMS-compliant documentation, and collaborate with an interdisciplinary team to achieve patient goals.

Qualifications

• Bachelor of Science in Physical Therapy from an accredited program.
• Active Physical Therapy license in state(s) of practice.
• Ongoing continuing education and professional development to maintain licensure and certification.
• Assume responsibility for ongoing continuing education and professional development to maintain current licensure and certification.

Responsibilities

• Provide physical therapy services to residents per standards of practice and departmental procedures.
• Adhere to documentation and billing procedures for therapy services.
• Develop treatment plans with short- and long-term goals and methods to achieve them.
• Communicate evaluation findings, plans, and recommendations to residents and interdisciplinary team.
• Complete compliant documentation in the CMS-mandated NetHealth EMR system.
