Job summary

Evolve Therapy Services is seeking a compassionate Speech Language Pathologist for an in-house Skilled Nursing Facility in Olivia, MN. This full-time position offers relocation bonus and tuition reimbursement, with additional options for Part Time or PRN scheduling.

The role focuses on evaluating residents, planning treatment, documenting in NetHealth, and coordinating FEES/MBS protocols, while promoting growth within the organization.

Qualifications

• Master's degree in Speech Language Pathology.
• Certificate of Clinical Competency.
• Active SLP license in state(s) of practice.
• Ongoing continuing education and professional development to maintain licensure and certification.

Responsibilities

• Provide quality speech therapy services and initiate referrals when appropriate, in accordance with standards and procedures.
• Evaluate and obtain data necessary for treatment planning and implementation.
• Develop a treatment plan with target list, short-term and long-term goals.
• Instruct patients, families, and staff in speech language procedures to enable carryover in daily activities.
• Complete CMS-compliant documentation in NetHealth EMR systems.
• Oversee FEES/MBS protocols and coordinate site programs.
• Participate in screening protocols to ensure resident needs are met timely and with clinical excellence.
