Job summary

Evolve Therapy Services, LLC is seeking a Speech Language Pathologist for an in-house Skilled Nursing Facility in Olivia, MN. This full-time role (also offering Part Time or PRN options) supports CMS-driven care and includes a relocation bonus and tuition reimbursement.

The SLP will plan, direct, and deliver speech-language pathology services, ensuring compliance with federal, state, and local standards while maintaining high-quality resident care.

Qualifications

• Master's degree in Speech-Language Pathology, CCC required.
• Certificate of Clinical Competency (CCC) required.
• Active SLP license in state(s) of practice required. Ongoing CE for licensure.

Responsibilities

• Provide quality speech therapy services and initiate referrals per standards and productivity targets.
• Evaluate and obtain data necessary for treatment planning and implementation.
• Develop a treatment plan with target lists, short-term and long-term goals.
• Instruct patient, family, and staff in speech-language procedures for carryover in daily activities.
• Complete CMS-compliant documentation in NetHealth EMR.
• Oversee FEES/MBS protocols and coordinate site programs.
• Participate in resident needs screening to ensure timely, clinical Excellence.
