Job summary

Evolve Therapy Services is seeking a Speech Language Pathologist for an in-house Skilled Nursing Facility in Olivia, MN. This is a full-time role with options for Part Time or PRN, plus relocation bonus and tuition reimbursement.

Responsibilities include delivering therapy, data collection for treatment planning, training staff, and CMS-compliant NetHealth EMR documentation. Must hold CCC and a valid SLP license in-state.

Qualifications

• Master's degree in Speech Language Pathology.
• Certificate of Clinical Competency.
• Active SLP license in state(s) of practice.
• Continuing education and professional development to maintain licensure.

Responsibilities

• Provide quality speech therapy and initiate referrals as appropriate.
• Evaluate and collect data for treatment planning and implementation.
• Develop treatment plans with target, short- and long-term goals.
• Instruct patients, families and staff to ensure carryover of therapy.
• Complete CMS-compliant documentation in NetHealth EMR.
• Oversee FEES/MBS protocols and coordinate site programs.
• Participate in resident screening to ensure needs are met timely and excellently.

Education

Master's degree in Speech Language Pathology
