Job summary

Evolve Therapy Services is seeking a Speech Language Pathologist for an in-house Skilled Nursing Facility in Kentucky. This part-time or PRN position requires a Master's degree in Speech Language Pathology, and an active SLP license in the applicable states, plus CCC and ongoing CEU requirements.

Responsibilities include providing quality therapy, data collection for treatment planning, developing plans with measurable goals, and ensuring CMS-compliant documentation in NetHealth EMR.

Qualifications

• Master's degree in Speech Language Pathology.
• Certificate of Clinical Competency.
• MUST have active SLP license in state(s) of practice.
• Ongoing continuing education and professional development to maintain current licensure and certification.

Responsibilities

• Provide quality speech therapy services and initiate referrals as appropriate.
• Evaluate and obtain data for treatment planning and implementation.
• Develop treatment plans with target list and short/long-term goals.
• Instruct patients, families, and staff in speech-language procedures for carryover.
• Complete CMS-compliant documentation in NetHealth EMR.
• Oversight of FEES/MBS protocols and coordination of site programs.
• Participate in screening protocols to meet residents' needs with clinical excellence.

Education

Master's degree in Speech Language Pathology

Certificate of Clinical Competency

Active SLP license in state(s) of practice
