Job summary

Evolve Therapy Services is seeking a Speech Language Pathologist for an in-house Skilled Nursing Facility. This PRN position requires providing high-quality speech therapy, documenting according to CMS CMS expectations, and coordinating FEES/MBS protocols with the site team.

Requirements include a Master’s in Speech Language Pathology, CCC, and an active SLP license in the practice states. Ongoing CE credits and documentation in NetHealth EMR are essential for compliance and resident care

Qualifications

• Master's degree in Speech Language Pathology.
• Certificate of Clinical Competency (CCC).
• Active SLP license in state(s) of practice.
• Ongoing continuing education and professional development.

Responsibilities

• Provide quality speech therapy services and initiate referrals when appropriate, in accordance with standards and productivity.
• Evaluate and obtain data necessary for treatment planning and implementation.
• Develop a treatment plan with target list, short-term and long-term goals.
• Instructs patient, family, and staff to carry over remediation and goals in daily activities.
• Complete compliant and medically necessary CMS documentation in NetHealth EMR.
• Oversight and implementation of FEES/MBS protocols with site coordination.
• Participate in screening protocols to ensure timely and excellent resident needs assessment.
