Evolve Therapy Services is hiring a Speech Language Pathologist for an in-house Skilled Nursing Facility. The role requires providing comprehensive speech therapy services, evaluating residents, and developing individualized treatment plans within CMS guidelines.

Active SLP licensure, a CCC, and ongoing continuing education are required. Responsibilities include documentation in NetHealth EMR and coordination of FEES/MBS protocols at the facility.

Qualifications

• Master's degree in Speech Language Pathology.
• Certificate of Clinical Competency.
• Active SLP license in practice states.
• Ongoing continuing education to maintain licensure.

Responsabilités

• Provide quality speech therapy services and initiate referrals.
• Evaluate and obtain data for treatment planning.
• Develop a treatment plan with targets and goals.
• Instruct patient, family, and staff for carryover of therapy.
• Complete CMS-compliant documentation in NetHealth EMR.
• Oversee FEES/MBS protocols and coordinate site programs.
• Participate in screening protocols for resident needs.
