Job summary

Evolve Therapy Services, LLC in Omaha, NE is seeking a Physical Therapy Assistant to provide in-house services at a Skilled Nursing Facility under the supervision of a Physical Therapist. This opportunity supports Full Time, Part Time, or PRN schedules.

The role emphasizes screenings, treatment planning, clinical documentation, and delivering both individual and group therapy to residents to improve mobility, strength, balance, and gait.

Qualifications

• The candidate possesses an Associate's degree in Physical Therapy from an accredited program.
• Must hold an active Physical Therapy Assistant license in applicable states.
• Commitment to ongoing continuing education to maintain licensure.

Responsibilities

• Provide physical therapy services to residents under the direct supervision of the Physical Therapist.
• Conduct screenings of residents to determine need for intervention or treatment.
• Assist in evaluating residents under the direction of the Physical Therapist.
• Assist in creating target lists, short- and long-term goals, and treatment plans.
• Deliver individual and group treatments focusing on strength, mobility, gait, balance, and modalities.
• Monitor residents' responses and modify interventions in consultation with the Physical Therapist.
