Creative Solutions in Healthcare, Inc. is seeking a compassionate Physical Therapy Assistant (PTA) to join our in-house rehab team. The PTA will provide person-centered care to improve mobility, strength, and quality of life for residents under physician orders and PT supervision.

The role emphasizes collaboration with an interdisciplinary team, thorough documentation, and ongoing patient education. In-house rehab and flexible schedules are highlighted benefits in a supportive facility setting.

Qualifications

• Active Texas PTA license in good standing.
• SNF or long-term care experience preferred.
• Current knowledge of physical therapy treatment practices.
• Experience with Net Health Electronic Documentation System preferred.
• Strong communication and teamwork skills.
• Ability to manage patients with varying personalities and needs.

Responsibilities

• Administer prescribed physical therapy treatments to relieve pain and improve function.
• Provide skilled PT services under supervision of a Physical Therapist.
• Relay information and referrals to the Physical Therapist.
• Implement and modify care plans under PT supervision.
• Measure and record motor function, strength, and muscle performance.
• Monitor patient response and adjust interventions with the team.
• Provide education to patients, families, and caregivers.
• Ensure thorough timely documentation per policies and regulations.
• Attend meetings as designated by the Director of Rehab.
