Job summary

Autumn Leaves Nursing & Rehab Center in Texas is seeking a Physical Therapist Assistant (PTA) to join our in-house rehabilitation program and deliver high-quality post-acute care.

Responsibilities include assisting with rehabilitation plans, coordinating with the care team, and communicating effectively with patients and families. Texas PTA license required; experience in SNF/long-term care is preferred. Competitive benefits accompany the role.

Qualifications

• PTA – Physical Therapist Assistant in the State of Texas.
• Experience in Skilled Nursing, Healthcare (Long Term Care) or Veterans Home preferred.
• Experience with EMR.
• Truly cares for people and will go above and beyond to provide outstanding patient care.

Responsibilities

• Assist with facilitation of rehabilitation of patients with emotional, physical or mental disabilities
• Assist with implementation of physical therapy care plans
