Job summary

Evolve Therapy Services, LLC is seeking a dedicated Physical Therapist for an in-house Skilled Nursing Facility in Hadley, MA. This full-time or PRN position focuses on planning, directing, and delivering comprehensive physical therapy to residents, with an emphasis on quality care and compliance with standards.

Responsibilities include assessments, treatment planning, documentation, and interdisciplinary collaboration to optimize resident outcomes.

Qualifications

• Must hold an active Physical Therapy license in state(s) of practice.
• Ongoing continuing education to maintain licensure and certification.
• Commitment to high-quality resident care and adherence to standards.

Responsibilities

• Provide physical therapy services to residents following established standards and procedures.
• Maintain accurate documentation and billing per CMS/Company requirements in NetHealth EMR.
• Assess, plan, implement, and follow up on resident therapy programs.
• Screen residents to determine need for intervention and treatment.
• Develop and communicate treatment plans with measurable goals.
