Evolve Therapy Services is hiring a Physical Therapist for an in-house Skilled Nursing Facility located in Hadley, MA. The role includes planning, directing therapy services and ensuring quality resident care.

Qualifications

• Bachelor of Science in Physical Therapy from an accredited program.
• Must have an active Physical Therapy license in state(s) of practice.
• Ongoing continuing education and professional development to maintain licensure and certification.

Responsibilities

• Provide physical therapy services to residents in accordance with established standards of practice and department procedures.
• Adhere to documentation and billing procedures for physical therapy services.
• Evaluate residents to determine needs and develop short- and long-term treatment goals.
• Develop treatment plans and communicate findings to residents, families, and the interdisciplinary team.
• Complete compliant and medically necessary CMS documentation in NetHealth EMR systems.
