Evolve Therapy Services is hiring a Physical Therapist for an in-house Skilled Nursing Facility located in Hadley, MA. The position is available as Full Time or PRN, with responsibilities to plan, implement, and direct physical therapy services to residents in accordance with applicable standards and CMS requirements.

The role requires a Doctor of Physical Therapy or B.S. in Physical Therapy, active PT licensure, and commitment to ongoing professional development.

Qualificações

• Minimum of Bachelor of Science Degree in Physical Therapy from an accredited program.
• Must have an active Physical Therapy license in state(s) of practice.
• Ongoing continuing education and professional development to maintain current licensure and certification.

Responsabilidades

• Provide physical therapy services to residents in accordance with established standards of practice and procedures.
• Adhere to documentation and billing procedures for physical therapy services.
• Evaluate and develop treatment plans with short and long-term goals and communicate findings to residents and the interdisciplinary team.
• Conduct screenings to determine need for intervention and plan accordingly.
• Complete compliant and medically necessary CMS documentation in NetHealth EMR.
